CRRT抗菌药物剂量优化参考
结合 CRRT 剂量提示抗菌药暴露不足、蓄积和 TDM 需求。
CRRT下药物清除受effluent剂量、滤器寿命、残余肾功能、蛋白结合率、ECMO和感染严重度影响，固定剂量表不应机械套用。
注意事项：本工具仅用于CRRT抗菌药剂量方向核对，不生成医嘱；具体剂量请由临床药师结合PK/PD、TDM和药敏MIC制定。
